Please be advised, Azimuth will move all existing members to our new network effective 12/15/2019. PLEASE, we request that members use the UHCG Network. If you need to access the Network prior to receiving your new ID card with the UHCG logo, please note you can search for providers and facilities on our website. Your ID number will not change.
